Practice owners know the balancing act: on the one hand, the legal obligation to maintain impeccable hygiene — RKI recommendations, TRBA 250, the Medical Devices Operator Regulation, the hygiene plan, documentation. On the other, the day-to-day reality of fully booked consulting hours, tight scheduling and a practice team with no capacity for thorough daily cleaning. Add to this inspections by the Health Authority, hygiene audits by the Medical Association or external certification bodies.
A standard office cleaning is not sufficient here. Anyone who underestimates the fact that contact surfaces, sanitary areas and waiting rooms in a practice carry a completely different infection risk than an administrative office will sooner or later run into problems: findings during inspections, patient complaints, and in the worst case, notifiable infection events. Many practices make do with general cleaning companies whose staff neither understand the difference between wipe disinfection and scrub-wipe disinfection nor can comply with the disposal regulations for Category B waste. This is no minor offence — it is a genuine liability issue for the practice owner.
Then there is the time that proper documentation consumes: the hygiene plan needs to be kept up to date, cleaning records need to be maintained, surface disinfection protocols need to be filed — all alongside patient care, billing, quality management and practice IT. Many medical secretaries and dental assistants find that cleaning documentation is the part of hygiene work most likely to slip. And that is exactly where inspections strike: missing signatures on the disinfection schedule, a blank entry for the previous week, a log that has carried the same handwriting for months. These are the findings that accumulate and, in the worst case, result in corrective orders or fines.
Our Solution
Kleibrink deploys staff specifically trained for medical environments who understand the differences between waiting rooms, treatment rooms, sterilisation areas and sanitary facilities — and who use correspondingly different cleaning and disinfection products. We work to your practice-specific hygiene plan, maintain written cleaning and disinfection records, and are well versed in the typical inspection points of the local health authority and KV audits.
Our teams arrive after practice closing time — in most cases in the evening between 18 and 22 o’clock, and also early in the morning for day clinic operations. Key handover, alarm system activation and coordination with the practice team are managed by a dedicated site manager, not by rotating contacts. For surface disinfection we use agents listed by the RKI (VAH list), and for waste disposal we observe categories A, B and C (AS waste code 18 01 04 or 18 01 03). And when the health authority comes, you can present our documentation in full.
We consistently differentiate between the risk zones of a practice. A waiting room with normal public traffic requires different products and frequencies from a treatment room in gynaecology or dermatology, from a procedure room with open wounds, and from the sterilisation area. Our cleaning team works with colour-coded wiping systems (a four-colour system based on HACCP logic adapted for practices): blue for office and administrative areas, yellow for sanitary facilities, red for areas at high risk of contamination, green for the tea kitchen and staff break area. Cloths and mops are changed per room, not per floor — this reliably prevents cross-contamination. In addition, our staff undergo annual training on updated RKI recommendations, KRINKO circulars and new VAH listings. Frequently Asked Questions
What does TRBA 250 mean and why is it relevant?
The Technical Rule for Biological Agents TRBA 250 governs infection control in medical facilities. It prescribes, among other things, how surfaces, work equipment and linens must be cleaned and disinfected. Our staff are trained in TRBA 250 and the RKI recommendations.
What is B-waste and how do you dispose of it?
B-waste (waste code 18 01 04) covers waste that is not subject to reporting obligations but may be contaminated with blood, secretions or excretions — for example wound dressings, single-use syringes without needles, swabs. We dispose of it in the designated tear-resistant containers, kept separate from general household waste.
Do you handle instrument reprocessing?
No. The reprocessing of medical devices is the responsibility of your practice team or an approved reprocessing unit. We clean surfaces and rooms — not instruments.
When do you clean — before or after consulting hours?
Typically after the practice closes, usually between 18 and 22 o’clock. Where day clinic operations or specific requests apply, cleaning can also be carried out in the morning before opening. The key point is that disinfectants must have their full contact time before the next patient enters the room.
Do you have references from the medical sector?

What happens during an unannounced inspection by the health authority?
